Proof of Income

  • Employed by others, taxes withheld, not an owner of the business, receiving a pay stub or earnings statement
  • A pay stub or earnings statement from the employer
  • Customers that can only access their pay stub on their company intranet must provide:
  • A copy of the pay stub and
  • Proof of receipt of that specific pay stub in the form of a bank statement/transaction summary or live/cancelled check

  • Business Owner / All Self-employed / 1099 employees of a company
  • Most recent three months consecutive complete personal bank statements from the same bank
  • If 1099 employee for a company may instead provide:
  • Cancelled checks (front and back) from prior three months

  • Business Owner (PayPal based) - Provide both of the following:
  • Most recent three months PayPal statements, accessed from an internet PC at the store and
  • Most recent three months personal bank statements showing matching deposits

  • Rental Income
  • Most recent two years professionally prepared and signed tax returns (form 1040 and Schedule E)

  • Military
  • Military Leave and Earnings Statement

  • For the following nine income types provide two documents:
  • One document from Category A: Proof of Eligibility for income proving they are eligible to receive that income AND
  • One document from Category B: Proof of Receipt of income proving they are actually receiving that income

  • Category A: Proof of Eligibility (provide one of the following, not required to be dated within 30 days of purchase, must show amount of award):
  1. Retirement / Pension1
    • Most recent eligibility letter from employer with amount and payment frequency
  2. Social Security (SSI/SSB)1
    • Most recent eligibility letter from Social Security Administration stating amount and payment frequency
    • Note: The SS eligibility letter is typically sent once a year at the beginning of the year
  3. Investment Income (Structured Settlements, Annuities, etc.)
    • Most recent award letter or court document stating amount to be received
    • Note: If no documentation for proof of eligibility is available for this income source only, partner with the financial institution
  4. Long Term Disability Insurance
    • Most recent award letter stating disability status and amount received
  5. Public Assistance
    • Most recent award letter stating award status and amount received
  6. Alimony2
    • Complete divorce decree / court order showing amount and payment frequency
  7. Child Support2
    • Complete divorce decree / court order / custody agreement showing amount and payment frequency
  8. Separate Maintenance2
    • Complete separation agreement / court order showing amount and payment frequency
  1. If the customer just began receiving Retirement or Social Security partner with the financial institution making the offer.
  2. Alimony, child support, or separate maintenance need not be revealed if you do not wish to have it considered as a basis for repaying this obligation
  • Category B: Proof of Receipt (In addition to a document from Category A above, provide one of the following dated within 30 days of purchase):
  • Most recent monthly statement from the income source (e.g. Social Security Administration, monthly statement from government agency or courts, etc.)
  • Most recent three months consecutive complete personal bank statements or pre-paid debit card statements showing deposits that match the award letter, court order, or court-issued document
  • Cancelled checks (front and back) from prior three months
